Dr. Loxley attended the University of Alabama at Birmingham’s School of Dentistry, graduating in 1994. Serving two tours abroad in the U.S. Army’s Dental Corps. from 1994-2004 completing her service as a Major. Selected for the military’s intensive Endodontic Specialty training program in 1998, Dr. Loxley earned her specialty certificate in Endodontics at the Army’s Tingay Dental Clinic and the Medical College of Georgia. She completed her residency program in 2000. Dr. Loxley started her Endodontic practice in Bend, Oregon in 2004 and recently sold her practice due to her autistic son’s needs, relocating here in Sonoma County. She loves spending time with her four children, scuba diving, reading, and traveling. She is a member of the California Dental Association, American Dental Association, American Association of Endodontics and the Redwood Empire Dental Society.

Dr. Jared Fortman
Jared R. Fortman, DDS
Dr. Fortman grew up in suburban Kansas City, Missouri where he attended The University of Missouri-Columbia for his undergraduate studies and The University of Missouri-Kansas City-School of Dentistry for his pre-doctoral training. He started his general practice in 1996 and after 5 years was chosen to join a 3 year post-graduate residency by Louisiana State Universitys Endodontic Department in New Orleans. Upon completion in 2004 Dr. Fortman, his wife and their 2 sons moved to Santa Rosa to begin their lives in California and start his private endodontic practice. He took the reins from the first endodontists in Santa Rosa and never looked back. “This is a great city in which to live and work. I feel fortunate that the people in this community have accepted me so graciously.”
Dr. Fortman continues to stay at the forefront of his profession with his main passions being retreatment of failed root canals and pain control. He is often heard saying, “A root canal should be boring. If you are bored then it didnt hurt and everything went as planned!” Dr. Fortman is a member of The Redwood Empire Dental Society, The California Dental Association, The American Dental Association, The California State Association of Endodontists and The American Association of Endodontists.
Dr. Fortman spends much of his personal time mountain biking with his sons, coaching their baseball teams, supporting school functions and relaxing with his wife Kelly.

Dr James Simonds
Dr. James Simonds is a practicing periodontist in Santa Rosa, California. After acquiring a BA in philosophy from Boston University and a D.M.D. from Tufts University School of Dental Medicine in Boston, he served at the National Naval Dental Center in Washington, D.C.. Dr. Simonds did his post-doctoral periodontal residency at Louisiana State University in New Orleans and then took a position as Assistant Professor in the Department of Periodontics at the University of the Pacific School of Dentistry in San Francisco. During his 4 years as a full-time faculty member, he also consulted for the Veterans Administration Hospital in Livermore and the Northern California Dental Hygiene Association. Dr. Simonds was also on the faculty of the Western Society of Periodontology Study Clubs.
Dr. Simonds is the author of several publications including the Practical Periodontics text, a periodontal patient manual Saving Your Smile, and he wrote a series of articles for the Journal of Practical Hygiene. He has given over 100 presentations nationwide. Dr. Simonds is the founder of Continental Education that specializes in continuing dental education and consulting for dentists.
In addition to his practice, he works to develop novel techniques in the area of periodontal plastic surgery and has conducted research focused in this area.
Dr. Simonds is a Past-President and current Editor for the Western Society of Periodontology and a Past-President and current Editor for the Redwood Empire Dental Society. He lives with his wife Dana, a Sonoma County Superior Court Judge, and has two children Daniel and Michelle.

Dr. Paul Steigerwald
Dr. Steigerwald received his dental degree from Ohio State University in 1984; and his certificate in periodontics in 1986 from Fairleigh Dickinson University. Dr. Steigerwald began placing dental implants in 1988 and to date has placed over 4000 implants.
Because of his many years of experience and affiliation with professors and academic institutions, Dr. Steigerwald has been involved in many of the techniques and innovations that are commonly used for successful implant dentistry today.
Dr. Steigerwald is a continuing education provider for the California Dental Association and the director of a local study club for dentists that focuses on diagnosis and treatment planning of complex restorative cases.

Dr. Justin Gee
Justin Gee, DMD, MSD
Dr. Gee received his bachelor’s degree in chemical engineering from University of California, Los Angeles in 1998. He then got his dental degree and specialty in periodontics at Boston University Goldman School of Dental Medicine in 2004 and 2007 respectively. He has practiced periodontics since graduating in Sacramento, California and Olympia, Washington before coming to Santa Rosa. He fell in love with the area and the nice people living here.
Dr. Gee regularly attends continuing education (CE) courses to update on latest technology and advances in the world of periodontics.
On his free time, he tries to stay active with mountain biking, hiking, and snowboarding. He and his wife love exploring Sonoma County. They enjoy spending much time with family and friends. He finds time to go on mission trips with his church group to provide dental care to those in need. He also participates in local charity events such as “Give Kids a Smile.”

Andrea Jordan, DDS
The blend of art and science first drew Santa Rosa native Dr. Andrea Jordan into the field of prosthodontics. Dr. Jordan earned her Bachelor’s degrees in Microbiology and Biochemistry at the University of California- Irvine achieving Honors distinction. She completed her Doctor of Dental Surgery at New York University (NYU) where she was honored with the Bernard Rudner Award for Clinical Excellence. At NYU, she also completed her postdoctoral specialty training in Prosthodontics, serving as chief resident and being distinguished with The Kenneth Adisman Award, given to the third year resident outstanding in their class. Following her training at NYU, she completed a fellowship in Maxillofacial Prosthodontics at the UCLA Medical Center, where she successfully completed her American Board of Prosthodontics (ABP) Examination, earning Diplomat status with the ABP and fellowship with the American College of Prosthodontics. Her extensive training allows her to treat congenital and acquired facial defects as well as complex reconstructive prosthodontics.

Dr. Chavezs Oral Surgery interests include treating dental phobic patients, implant surgery, bone grafting, trauma, TMJ, and facial reconstruction. Outside of Oral Surgery he enjoys spending time with his family, running, sports and exercising. He completed his undergraduate degree at Sonoma State and his dental degree from University of California, San Francisco. He then went on to complete his Oral & Maxillofacial Surgery residency at University of Pacific/Highland Hospital.
Dr. Mogannam is very excited to be able to practice in his home town of Santa Rosa where his family roots are deep. He is married to Cindy, a Dietitian, and together they have two children, Gabrielle and Peter. Dr. Mogannams Oral Surgery interests include treatment of children and young adults, implant surgery, bone grafting, and facial reconstruction. Bernstein Orthodontics – Rael Bernstein, DDS, MS
Dr. Bernstein was born and raised in South Africa where he attended the prestigious Wits Dental School finishing top of his class. He then went to complete his orthodontic specialty residency at the State University of New York at Buffalo in 1998. Following his residency, Dr. Bernstein accepted a fellowship position at the University where he became a clinical assistant professor at the orthodontic department. Having a yearning for a more similar climate to South Africa, and tired of shoveling snow, he relocated to Southern California where he met his wife Debbie, also a dental specialist (prosthodontist). Dr. Bernstein relocated to Santa Rosa in 2002 to get away from the hustle and bustle of Southern California.
As a former orthodontic patient, he not only understands his patients needs and concerns but also the wonderful benefits of a healthy bite and broad smile.
Dr. Bernstein is committed to excellence in orthodontics. He is also the only invited guest orthodontist to teach at the Hygiene and Dental Assisting School at the Santa Rosa Junior College. He has lectured nationally on self-ligation and early interceptive treatment to orthodontists and dentists.
He recently served as president of our local (Sonoma, Mendocino, Lake Counties) Redwood Empire Dental Society for 2009, and is also an active member of the California Dental Association, the American Dental Association, the California Association of Orthodontics, the Pacific Coast Society of Orthodontics, and the American Association of Orthodontics.

An active member in both, the Pacific Coast Society of Orthodontists and the American Association of Orthodontists, Dr. Bales is also Past President of the Marin County Dental Society, a component of the California and American Dental Associations. Dr. Tom was honored in March 2004 when the new UCLA Orthodontic Clinic was dedicated the DR. THOMAS R. BALES ORTHODONTIC CLINIC at UCLA, for his leadership, dedication and philanthropy to the Orthodontic Residency Program there.
Dr. Kathleen M. Bales was born in Novato, California. She attended the University of Colorado at Boulder for her undergraduate education, majoring in Biology. She received her Doctor of Dental Surgery degree in 2003 from the University of the Pacific School of Dentistry in San Francisco, California, where today she enjoys teaching Invisalign part time to the dental students.
Dr. Katie completed her orthodontic residency in 2006 at the University of California at Los Angeles, where she was chief resident and simultaneously earned a Master of Science degree. Her research was focused on the orthodontic treatment of cleft palate patients.
Dr. Katie is a member of the American Association of Orthodontists, American Dental Association, California Dental Association and the Pacific Coast Society of Orthodontics as well as the local dental societies of Marin and Sonoma County. Like her father, she has the distinction of being a board-certified orthodontist and a diplomate of the American Board of Orthodontics.
